Os respons\u00e1veis pelas crian\u00e7as dever\u00e3o enviar e-mail para\u00a0dedicss@unicamp.br<\/a> confirmando o interesse pela vaga antes da efetiva\u00e7\u00e3o da matr\u00edcula, conforme os itens 02 e 03, at\u00e9 o dia\u00a0 14\/04\/2026<\/span>, sob pena de perda da mesma em prazo posterior ao determinado.<\/strong><\/li>\n
  2. ATEN\u00c7\u00c3O<\/span>:\u00a0As matr\u00edculas ser\u00e3o realizadas presencialmente na SECRETARIA EDUCACIONAL no endere\u00e7o, Rua Carlos Chagas 351, conforme indicado na Lista de Contemplados.<\/strong><\/li>\n
  3. Acessar o link para preencher o Formul\u00e1rio de Matr\u00edcula<\/a> \u2013 O processo de matr\u00edcula dever\u00e1 ser realizado pelo respons\u00e1vel com v\u00ednculo institucional, sendo este o respons\u00e1vel legal pela vaga.<\/strong><\/li>\n
  4. No momento da matr\u00edcula (que est\u00e1 indicado na lista de contemplados<\/span>), dever\u00e3o ser entregue: Formul\u00e1rio de Matr\u00edcula, devidamente preenchido e assinado e os demais documentos listados abaixo<\/u><\/strong><\/li>\n<\/ol>\n

      2. <\/strong>Atestado M\u00e9dico\u00a0para todas as crian\u00e7as;<\/u><\/strong><\/li>\n
      3. Relat\u00f3rio M\u00e9dico espec\u00edfico detalhado e atualizado, quando necess\u00e1rio, indicando aten\u00e7\u00e3o e cuidados especiais para alergias, restri\u00e7\u00e3o alimentar e outros;<\/strong><\/li>\n
      4. <\/strong>Atestado M\u00e9dico para as crian\u00e7as\u00a0com necessidades especiais (Laudo M\u00e9dico ou em investiga\u00e7\u00e3o);<\/u><\/strong><\/li>\n
      5. C\u00f3pias do RG da crian\u00e7a ou Protocolo de Retirada e CPF;<\/u><\/strong><\/li>\n
      6. C\u00f3pia dos relat\u00f3rios pedag\u00f3gicos da crian\u00e7a, que foram emitidos pelas institui\u00e7\u00f5es escolares anteriores a DEdIC<\/b>;<\/span><\/b><\/u><\/strong><\/li>\n
      7. Formul\u00e1rio de Matr\u00edcula preenchido e assinado (acessar o link<\/a> e preench\u00ea-lo);<\/u><\/strong><\/li>\n
      8. SOMENTE PARA AS CRIAN\u00c7AS DO PRODECAD<\/u><\/span> \u2013 c\u00f3pia do atestado de matr\u00edcula da Escola S\u00e9rgio Pereira Porto<\/span>.<\/strong><\/li>\n<\/ol>\n<\/li>\n<\/ol>\n

        OBS.: Devido a restri\u00e7\u00e3o de acesso de pessoas nas unidades da DEdIC informamos que somente dever\u00e3o permanecer no espa\u00e7o do hall da secretaria, para efetiva\u00e7\u00e3o da matr\u00edcula, o respons\u00e1vel pela vaga.<\/span><\/strong><\/p>\n
